  • Nursing assessments: We conduct comprehensive assessments to guide holistic care. We collaborate with medical team to devise an ideal plan for ongoing chronic disease management.
  • Medication Management and Assessment: Effective medication management is crucial for maintaining optimal health. We assist formulation of personalised medication plans in collaboration with pharmacists/ doctors. Also, offer medication administration services to ensure adherence to the plan and medication regimen.
  • Complex Wound Care Assessment: Guided by evidence-based practise, we can provide wound care and also educate participants to manage their wounds independently. This is also inclusive of associated pain management. Regular wound reviews/ analysis ensures the progress is closely monitored and further specialist input sought if required.
  • Continence Assessment and Catheter Management: We believes that incontinence is not only a medical problem, it’s a matter of dignity and quality of life. Therefore, we provide continence assessment to explore with you, the bowel and bladder issues experienced, degree of incontinence, type of incontinence, frequency, side effects of medications etc. Hence, we create plan with you for your continence management. Indwelling catheters require management to minimise risk of infections and other associated complications, we will change your catheter and devise a plan to guide you with its management to minimise the risk of complications. Our staff will cater assistance as per your care plan.
  • Complex Complex Bowel Care Support and Care: -: We at EUU, provide complex bowel care with empathy and dignity. Through our expertise we understand that complex bowel issues are often linked with other chronic health conditions and inadequate nutrition. Therefore, we work with you to develop a care plan that addresses your complexity and uniqueness whilst maintaining your privacy and dignity. We provide this
    Stoma Management: Stoma is a surgically created opening usually in the abdominal area for elimination of urine or faeces. We provide care to maintain stoma hygiene, skin integrity, application and maintenance of appliances. Also, provided is education on maintaining a healthy stoma and early detection for any potential complications. We also assist in liaising with the stoma specialist to enable specialist support to address any issues.
  • Diabetes Assessment:Diabetes is a fastest growing chronic condition in Australia and affects every individual differently. Therefore, at EUU we formulate a plan with you in reference to the recommendation from your GP/ specialist or diabetes educator. We assist you to manage your diabetes and adhere to your diabetes management plan. Management can include glucose monitoring, medication administration, balanced diet and exercises.
  • Palliative Care:At EUU we understand the challenges and complexities in supporting your loved one with a terminal illness or in their terminal stage of life. Therefore, in terminal phases we offer medication administration (oral or injectables), catheter care, stoma care, continence care, hygiene and skin care. Also offered is in home respite during the day or overnight.
